1. Print and Complete the Visa application form.

2. Include 2 photographs (2"x2"). Should face front, against a white or off-white background.

3. Include your passport.

4. Include your business check or money order payable to: Consular Assistance, Inc. Personal checks are accepted for regular processing only. (For appropriate amount and for processing times, see Moldova Visa Fees page)

5. Include a paid United States Postal Service Express Mail, FedEx, DHL, UPS envelope with your return address. We recommend using FedEx. If you do not have a FedEx account and would like to use our account, please call or e-mail us and we will give you our FedEx account number. The charge is $18 one way and $36 roundtrip.

6. Mail only with United States Postal Service Express Mail, FedEx, DHL, UPS to:

7. After mailing your documents, send to visa@moldova.org the following information: (a) mail carrier & tracking number; (b) number of passports mailed, (c) country or countries visa is required; (d) the amount of the check or money order.

IMPORTANT INFORMATION TO AVOID DELAYS

Avoid DELAYS to the issue of your visas by checking the following details carefully. Processing of your visa cannot commence until all documents are correctly submitted to the Embassy.

PASSPORT

You must enclose your passport. Please check that it is signed and valid at least for a further six months after the expiration of the visa. Make sure that there are blank VISA pages available.
